Patient Account Billing

RML is certified by Medicare, Medicaid and Blue Shield to perform the laboratory tests ordered by your physician.  We will bill any combination of these carriers at no additional charge to you when provided with adequate billing information.  If we do not receive complete billing information, we will send you a statement requesting you to call us.

BLUE SHIELD:  RML is a participating member with Blue Shield and accepts their reimbursement as payment in full.  You will be billed only for policy deductibles, copays and tests not covered by your policy after we receive Blue Shield payments or rejections.  We submit claims to all Blue Shield carriers when provided with the contract holder’s name and the entire contract number including any alpha prefix and a diagnosis provided by your doctor.

MEDICAID:  RML is a Michigan Medicaid participating provider.  If you are covered by Medicaid, you will be billed only for tests not covered by Medicaid for which you received prior notification or when no payment or rejection is received during the first 60 days from billing.  We submit claims to Medicaid and Medicaid HMO’s when provided with the beneficiary’s identification number and the name of their HMO and a diagnosis provided by your doctor.

MEDICARE:  RML is a Medicare participating provider.  If you are covered by Medicare, you will be billed only for deductibles or for tests not covered or rejected by Medicare for which you received prior notification after we receive a Medicare payment, denial or deductible notice.  You may be asked to sign an Advance Beneficiary Notice for any procedures that we believe Medicare will not cover.  We submit claims to Medicare when provided with your complete identification number and a diagnosis provided by your doctor.

COMMERCIAL INSURANCES:  RML participates with many commercial insurance carriers.  You will be billed only for policy deductibles, copays and tests not covered by your policy after partial payments or rejections are received.  We submit claims to all commercial carriers if we are provided with the appropriate insurance information, including the policyholder name and identification number, the insurance company’s address, and a diagnosis provided by your doctor.

If your physician collects your laboratory specimen, he or she will complete an RML test requisition with your name, address, social security number, birth date, address, telephone number and complete insurance information, along with a diagnosis for each test ordered.  Please remember to keep your physician updated with any new insurance coverage.  RML bills your insurance when your physician provides adequate and appropriate billing information.

If RML collects your laboratory specimen at one of our Patient Service Centers, our registration personnel will obtain your insurance information to ensure our records are accurate and current.  Please remember to bring all your insurance identification cards with you each time you visit our laboratory.

Generally, you will not receive a bill from RML until your insurance makes a partial payment, or when no payments are received during the first 60 days.  When your insurance is billed and a patient balance remains, you will receive an itemized bill from RML within one week from the date your insurance payment, denial or deductible was applied.  If a balance remains on your account after four weeks, we will send another statement to you. 

No Insurance Coverage:  RML will bill you within two weeks from the date of service.  If a balance remains on your account after four weeks, we will send another statement to you.  Payment plans are available by calling (269) 969-6161, extension 6987.
